1. "I have had about 3/4 T41P phones loose programming and default to factory mode."
Do you check the auto provisioning url in the webpage? Enter the webpage->Settings->Auto provision, check whether you filled a URL in the Server URL.
2. "Also when that happens all my other T41 and T46G phones go into No Service and I need to power all phones to get back to working status."
Do you check about your network connecting?

3. "No Service"
This means that you don't register an account successfully now.
