Typically this issue is seen when you do an attended or semi-attended transfer. Are you sure that you are performing a blind transfer?

In both blind and attended transfers the callerID being presented to the trasferee phone is handled by the PBX, so you should also reach out to your PBX provider and ensure transfer configuration and support is setup properly there too.
