Sounds more like your service provider changed something on your end and is not letting the call forward happen. Since you are not in control of the PBX you will need to contact the provider and see if they can assist you in troubleshooting this. I doubt it's anything to do with the phone itself. If you wanted to be really sure you could run a pcap trace on the phone, then try dialing in and see exactly what the SIP messages back and forth say when the call gets dropped. I suspect you would see the phone send a "Forward to ###" message to the PBX, and the PBX send back a rejection or failure for some reason.
