2. incoming call? Seems to be internal or external from reports
3. Barge is disabled
4. when you press headset key, the call still on speaker rather than headset? I haven't had a chance to try as the reports are after the fact.

This is a tough one to diagnose as it seems to be extremely random.

Please advise.

I know that the local "auto-answer" feature controls if the phone will answer every call. Yealinks also support auto-answer calls controlled by the caller via an addition to the SIP header of the invite:

same => n,SIPAddHeader(Alert-Info: Ring Answer)
same => n,SIPAddHeader(Call-Info:\;Answer-After=0);
same => n,SIPAddHeader(P-Auto-Answer: normal);

We use this to control custom "paging" scenarios. Not sure if you can run a long pcap of the SIP traffic, but looking at SIP invites in wireshark would tell you what was happening.
