Our system is setup that the carrier asks the hosted system every 15 minutes during the call, if the call is still active. the system then sends an invite to the phone and the phone replies. the system then confirms to the carrier and the call continues.

The problem was, that the phone didn't always reply to the systems request, and after three invites, the system advised the carrier that the call is not active. and the carrier dropped the call. and the phone still shows that the call is in progress. but it's actually been disconnected.

Good news is, that the issue resolved after upgrading to the new firmware:
