I was looking for the same thing, but I think that this is possible. As mentioned, usually you would register to a server. You should be able to do this peer-to-peer without a registration server.
Here is how to do this with Grandstream IP phones. I suspect that it may be possible to also do this with Yealink to Grandstream.
