Yes. It is strange in two user guide. But i think you can change to insert different port of RJ11(RJ9) in Plantronics CS60.
It has two RJ11(RJ9) ports, we connect to the Telephone Cable Port. (attachment)

Your connection is right. And your issue is strange.
Again, make sure your power of T28 is right.
And you have provided pacp trace to our support@yealink.com.
Our R&D colleague will help to solve it.

Ok, I use original power supply of T28 phone box.
Ok, I open a ticket and send pcap, syslog, etc..
