I saw messaging on this problem with headsets on the T-46 from 2015, but did not see a fix. I have bought 3 headsets to use with my T-46, the last being a Bose. Persons with whom I am on the phone reported the same loudness and shrillness and tininess problems with all three. Adjusting volume on the accessories does not fix the problem, and inasmuch as I saw this same messaging earlier, I hope it is a known problem.

I would do next steps: (in that order)
- upgrade the phone to the lastest available firmware
- reset the phone to factory default
- re-provisioning the phone
- use the G711 codec only
- test again
