I have submitted the requested troubleshooting data to Yealink support, and I will update this thread when I receive an instructive response that helps resolve this issue.
Hi

We are also experiencing this problem on:

T21P FW: 34.72.0.21
T21P FW: 34.72.0.75

We are NOT experiencing this on:

T20P FW: 9.70.0.140 (too scared to update)

The same physical switch is used on all three(3) the above scenarios without any change to config:

CISCO SRW224G4P FW: 1.1.2.0

How many T21 are deployed in your site?
How often the issue happens?

Hi James,

Yes we use LLDP to configure the VLAN on the WAN port. The phone then does DHCP on the Voice VLAN and get the provisioning URL via DHCP on the Voice VLAN.

We currently have about 25 X T20P. We bought 2 X T21P to start looking at replacing the T20Ps as needed because they are end of life.

They issue is reproducible and happens everytime without fail.

Can you get a pcap file and level 6 syslog when phone boot up?
With the two files we can know what packet phone send at the process and whether it receive a reply from switch.

You can connect a PC to phone PC port, go to web interface-> Network-> Advanced, enable span to PC, then start a pcap capture tool like wirehsark to get a capture.
To syslog, plesae set it to level 6 firstly, then after booting up process, export it.

Thanks for the files.
I have submitted them to our R&D and generally they will give a feedback next week. Any update will let you know.

From the pcap file, phone send LLDP packets to Cisco switch and receive LLDP packets from Cisco switch. But it seems that the LLDP pacekts phone received don't contain a VLAN informaton. (The VLAN ID is default 1)
Then phone send DHCP packet without a tag and receives no reply from server which cause the no ip address problem.

If possible can you also provide pcap/syslog/config.bin of T20? By comparing the two set files it's more easier to confirm the problem.
BTW, I think the phone can get an ip address if you configure a VLAN manually?

Please test this firmware:
