We are having Speed Dial keys that are programmed on T46 phones suddenly disappear. They are firmware version 28.80.0.60. A speed dial can be setup one day and then the next day or two it is gone.

I have seen this as well. In my case, it was just the entries on the first page. The Type revered to Line and the Value was blank, but the label intact.
I had both T46G's on our account have that happen on the same day. I noticed that my DHCP'd IP had changed, so I am attributing this to something on the provider's side, or to a reboot. I'm pretty sure that the provider (Vonage) upped the firmware version provisioned, so it could also be attributed to a firmware update.
What phone system do you use?
Is your phone system doing provisioning?
By default 3CX will blank out the DSS keys on the default template unless the keys are programmed into 3CX.
